    The assurance learning loop

    Findings lead to remediation, remediation leads to better controls, and every change shows up in ORS with its evidence attached.

    Six steps

    1. Finding

    A test, incident, supervisory comment or disposition produces a finding, attached to the TrustGraph object and the obligation it touches.

    2. Owner and plan

    The finding is assigned to a named owner with a remediation plan and a date. ORS reflects the open finding straight away.

    3. Remediation evidence

    Closure needs evidence, not a status change: the fix, the retest and who approved it.

    4. Control change

    If the finding revealed a weak control, the control itself is changed, versioned and re-tested, so the same failure is not found again elsewhere.

    5. Score movement

    ORS moves only when evidence is accepted. Every movement can be traced back to the finding and the evidence that caused it.

    6. Pattern

    Repeated findings across entities or controls are raised as a pattern for the board, not just closed one by one.
